is the Leduc Beaumont Devon Primary Care Network?
The Leduc
Beaumont Devon Primary Care Network (PCN) offers support to the patients of our
member physicians. This collaborative team approach allows for better access to
healthcare resources. Our PCN healthcare team is made up of nurses, dietitians,
an exercise specialist, registered pharmacist, registered psychologist, mental
health clinicians, referral coordinators and support staff. In addition to the
care you receive from your family doctor, programs and services offered at the
PCN include:

- · Smoking cessation
- · Weight management
- · Mental health
- · Geriatric consultation
- · Referral coordination
- · Exercise prescriptions
- · Cardiac rehabilitation program
- · Specialist clinics
- · After-hours clinics (Leduc and Beaumont)
