The Leduc Beaumont Devon Primary Care Network and the Faculty of Nursing PRIDE Research group at the University of Alberta would like to invite you to participate in a research study.
Why are we doing this research?
In older adults, Type 2 Diabetes Mellitus is a common and serious disease that often occurs alongside other chronic conditions.
Research has shown that management of diabetes and other chronic conditions leads to improved health and quality of life. New approaches are needed to better support older adults and their family members to manage chronic conditions at home.
Who can participate?
You may be eligible to participate if you are:
- at least 65 years of age
- diagnosed with Type 2 Diabetes
- have at least two chronic conditions in addition to diabetes. Some examples are, but are not limited to:
- High blood pressure
- Previous heart attack
- Previous stroke
- Poor kidney function
- Asthma or COPD
What do I need to do to participate?
All participants will take part in two interviews. One will be completed at the start of the study and the other at the end of the study (6 months later). Each interview will take about 2 hours and will focus on your health and use of health services. A research assistant from the University of Alberta will come to your home for both interviews.
What will I receive?
You will have a one in two chance of receiving a community-based intervention in addition to your usual diabetes services.
Usual Care Group
Other than the interviews at the beginning and end of the study, you do not need to do anything else to participate.
Participants will be invited to take part in a monthly 2-hour Diabetes Wellness Day group program offered by their local Diabetes Education Centre and a partnering community organization over 6 months.
Participants in the intervention group will also receive up to 3 in-home visits by a Registered Nurse and/or a Registered Dietitian from their local Diabetes Education Centre over 6 months.
What will I get for participating?
Every participant will receive a $25.00 gift card in appreciation of their time.
Your participation in the study will make an important contribution to health research in the care of older adults with type 2 diabetes and other chronic conditions.
