January is Alzheimer Awareness Month

During this year's Alzheimer Awareness campaign, the Alzheimer Society of Canada challenges all Canadians to rethink their perceptions of Alzheimer's disease and other dementias.

Life does not end when Alzheimer's begins.

People living with dementia can continue to participate in life and contribute to their communities even as the disease progresses. It's important to be there, for those who are #StillHere.

The Leduc Beaumont Devon Primary Care Network offers support for those with dementia or Alzheimer's through the Leduc Eldercare Consultation Team, which consists of (left to right) LBD PCN Geriatric Primary Care Nurse, Carla Moore and Care of the Elderly Physician, Dr. Trevor Byers.



The Leduc Beaumont Devon Primary Care Network provides a specialized geriatric assessment service for seniors who have chronic, complex health issues.  Our goal is to ensure seniors living in Leduc, Beaumont and Devon enjoy optimal health and quality of life while remaining in the community for as long as possible.

We accept referrals for the following conditions:


  • cognitive/behavioural concerns
  • functional decline (falls, mobility issues)
  • review of medications
  • capacity assessment


This program is by referral from a PCN physician. If you or your loved one needs support, please talk to your family doctor.
